1. Declaration of Health Care Coverage Form (Mandatory)
2. FSA/DCA Enrollment/Change Form (Mandatory)
Need to determine if you are participating or not in 2024 - Yes or No (If yes, how much total annual election)
3. Sworn Statement of Alternative Insurance/Cash in Lieu Form (Mandatory) - if you are not enrolled or not participating in the District's Health Plan for 2024 - Provide a copy of your insurance card also.
4. Enrollment Change Form BCBS of VT Form (Optional) - Mandatory Form only if New Enrollment or Dropping - Adding or Removing Family Members - Changing Plans
5. HRA Enrollment/Change Form (Optional) - Mandatory Form only if New Enrollment - Adding or Removing Family Members - Changing Plans
6. HSA Enrollment Form (Optional) - Mandatory Form only if New Enrollment - Currently Enrolled or Changing to the Silver CDHP Plan. HRA is still available with the Silver CDHP plan OR You may now choose an HSA with the Silver CDHP plan
7. Domestic Partnership Affidavit Form (Optional) - Mandatory Form only if Adding Domestic Partner to Health or Dental Coverage - Please review Criteria/information
8. Dental Enrollment Change Form (Optional) - only if New Enrollment or Dropping - Adding or Removing Family Members
9. Optional Life Enrollment Form (Optional) - Mandatory Form only if New Enrollment or Applying for Changes
10. Evidence of Insurability Form (Optional) - Mandatory Form only if New Enrollment or Applying for Changes with your Optional Life Policy
11. Vision Insurance Enrollment Form (Optional) - Mandatory Form only if New Enrollment or Changes with your Vision plan.

What is a Health Reimbursement Account? (HRA)
A Health Reimbursement Account (HRA) is money in an account that the BUUSD District gives to you as the employee, each calendar year, to cover eligible claims up to a certain amount, before you are responsible to pay your last dollar responsibility. HRA’s are only available to employees who receive their health care coverage from the BUUSD.

What is a Flexible Spending Account? (FSA)
A Flexible Spending Account (FSA) is your money you put into an account, bi-weekly out of your paycheck, pre-tax, to use to pay eligible out-of-pocket medical expenses.

What forms are mandatory if I do not want to change anything?
If I have the BUUSD Insurance plan(s) and I do not want to change anything, complete these forms:
- Declaration of Health
- FSA/DCA Enrollment/Change form
If I do not have BUUSD benefit plan(s) and do not want to change anything, complete these forms:
- Declaration of Health
- FSA/DCA Enrollment/Change Form
- Sworn Statement of Alternative Health Insurance Coverage (Both sides if apply, and provide a copy or your current insurance card)

Yes, this is for January 2024-December 2024, and every eligible employee needs to do this paperwork each Open Enrollment.

What is a Limited Purpose Flexible Spending Account? (LPFSA)
A Limited Purpose Flexible Spending Account (LPFSA) is your money you put into an account, bi-weekly out of your paycheck, pre-tax, to use to pay eligible out-of-pocket dental, and vision expenses only. This is available for when you have a Silver CDHP plan with an HSA.

Please complete and return all Mandatory Forms to the Human Resources Department by the end of the Open Enrollment period - November 06, 2023.
